Notes for Notice Letter to Employer
[1]  The date of the letter.
[2]  The name and address of the employer.
[3]  The plaintiff’s last name in the divorce case.
[4]  The defendant’s last name in the divorce case.
[5]  The type of trial court that granted your divorce (“Superior”, “Circuit”, etc.).
[6]  The name of the county where your divorce was granted.
[7]  The case number of your divorce.
[8]  The date your divorce decree was “entered” (generally, the date on the clerk’s file-stamp on your divorce decree).
[9]  The person to whom you are addressing the letter.
[10] Your former spouse’s name.
[11] The date the income deduction order was entered.
[12] Your name.
