Based on your failure to pay monthly rental for _, and your continued failure to timely cure this default after written notice thereof, you are hereby notified of my decision to terminate the Lease. Pursuant to the Lease, demand for possession is hereby made. You are to immediately vacate the Premises and surrender possession of the same to me.
