You should also address the letter specifically to the Judge’s secretary, law clerk, or staff attorney, and not directly to the Judge. [3] The plaintiff’s last name in the divorce case. [4] The defendant’s last name in the divorce case. [5] The type of trial court that granted your divorce (“Superior”, “Circuit”, etc.). [6] The name of the county where your divorce was granted. [7] The case number of your divorce. [8] The date your divorce decree was “entered” (generally, the date on the clerk’s file-stamp on your divorce decree). [9] The person to whom you are addressing the letter – probably not the Judge directly. See [2]. [10] Your former spouse’s name. [11] The date of your divorce; same as [8]. [12] Your former spouse’s name. [13] You will have to look up the paragraph number from the divorce decree or your settlement agreement that gives you the right to have the QDRO entered.
